FAQs About Washington State Nurse Delegation and Advocacy

In Washington, nurse delegation is a legal process that allows a registered nurse to transfer specific nursing tasks—like insulin injections or wound dressing—to trained caregivers in adult family homes. The RN assesses residents, teaches the task, confirms competence, and provides written delegation.

State law protects residents by ensuring skilled oversight for medical tasks. When a task falls outside a caregiver’s basic certification, only an RN can delegate it. This safeguards quality, maintains compliance for adult family homes, and offers families documented peace of mind.

Commonly delegated tasks include blood glucose checks, insulin administration, simple wound care, catheter or ostomy maintenance, basic tube feeding, and certain oral or topical medications. The RN reviews each resident’s condition to determine appropriateness and can withhold delegation if safety concerns exist.

For private-pay residents, Washington requires that the delegation fee be billed to the resident or family, not the adult family home. Our current rate is $150 every 90 days, covering assessments, training, documentation, and ongoing oversight visits.

Most caregiver trainings for a single delegated task can be completed during one onsite visit, often 30–60 minutes depending on complexity and learner experience. Additional time may be needed when multiple tasks are delegated or refreshers are requested.

Washington rules mandate that the delegating RN re-evaluate each resident and caregiver competency at least every 90 days. During these visits we verify documentation, observe technique, update care plans, and address any changes in condition or medication orders.
